How to Become a Contracted Provider
To become a participating provider, you must first sign a contract and complete the necessary paperwork, including a contract (PCP or Specialist), a W-9 form, and credentialing information.

Credentialing Information
- Participating providers can submit their credentialing information to the IPA of Georgia in one of two ways. The first is the Council for Quality Healthcare (CAQH), a free universal credentialing data source. CAQH is a non-profit alliance of health plans, networks and trade associations working together to simplify the credentialing process and to reduce paperwork and time spent by physicians and their office staff on certain administrative tasks. Providers submit all information just once to CAQH in a standardized application data format. The provider then authorizes the release of the data to the IPA of Georgia. Only those organizations authorized by the provider are able to receive the data. Automatic reminders are sent each quarter to update specific information as it becomes due. The second way for providers to submit their credentialing information is on a Georgia Uniform Healthcare Practitioner Credentialing Application Form. Forms were developed from the cooperative efforts of the Georgia Hospital Association, Georgia In-House Counsel Association, the Georgia Association of Medical Staff Services, the Georgia Association of Health Plans and the Medical Association of Georgia.
